Starting February 23, 2021 onwards, all General Information Sheets (GIS) and Notification Update Forms (NUF) submitted to the SEC by stock, non-stock and foreign corporations must now include the official and alternate e-mail addresses and cellphone numbers submitted by the corporation pursuant to SEC Memorandum Circular (MC) No. 28 s. 2020 (SEC MC 28-2020).
Failure to include the e-mail addresses and cellphone numbers shall render the GIS and NUF filed incomplete. The revised GIS and NUF templates may be downloaded here:
· stock corporations (GIS)
· non-stock corporations (GIS)
· regional operating headquarters, regional headquarters of multinational companies (GIS)
· regional operating headquarters, regional headquarters of multinational companies (NUF)
Reportorial requirements must be submitted through the SEC Online Submission Tool (SEC OST) starting March 15, 2021. OST Kiosks will be set up at SEC Offices from March 15 to December 15, 2021 to cater to companies and filers who have issues registering on the SEC OST, provided that proof that such issues exist must be provided. A screenshot of the error would suffice. Companies and filers may book an appointment online through the SEC Express Submission Assistance System.
